Description

The Supportive Housing Specialist is a vital part of the team at Del-Mor Dwellings, responsible for providing support to residents, meal planning and preparation, maintaining cleanliness in common areas, and documenting daily activities. This role also requires completing clear, accurate, and timely shift notes to ensure effective communication with the team and to document resident progress/changes and needs.

This position will rotate between 2 housing units in the city of Delaware and must have open availability for variable hours.

This position is ideal for someone who is dependable, organized, and compassionate. You will be directly involved in helping residents maintain their independence while ensuring a clean, safe, and welcoming environment. The ideal candidate will demonstrate empathy, attention to detail, and strong communication skills.

I. Primary Responsibilities

  • Resident Support & Relationship Building
  • Develop trusting, respectful relationships with residents to promote their independence and overall well-being.
  • Provide emotional support, guidance, and encouragement to residents in daily activities and challenges.
  • Encourage residents to participate in wellness activities and community-building opportunities.
  • Meal Planning, Preparation, and Budget Management
  • Maintaining Cleanliness of Common Areas
  • Daily Duties Checklist
  • Complete a required checklist of daily duties and responsibilities to ensure all tasks are carried out efficiently and consistently.
  • Completion of Daily Shift Notes
  • Accurately document daily shift notes detailing resident activities, interactions, and any concerns or observations during the shift.
  • Safety & Environment Management
  • Maintain awareness of resident and visitor activities on the property to ensure a safe and respectful environment.
  • Promote positive behavior and intervene when necessary, using sound judgment in line with established procedures.
  • Collaboration & Communication
  • Collaborate effectively with other Del-Mor staff members to coordinate resident care and ensure program objectives are met.
  • Maintain open lines of communication with guardians, case managers, and other service providers to ensure a coordinated approach to resident care.
  • Actively participate in regular team meetings and contribute to the development of program strategies and resident outcomes.
  • Role Model & Wellness Advocate
  • Serve as a positive role model for residents, demonstrating healthy habits and responsible decision-making.
  • Promote healthy living, encourage wellness, and support residents in making choices that improve their overall well-being.
II. Qualifications & Requirements

  • Experience & Education
  • Coursework or training in mental health, nutrition, social work, or a related field is a plus.
  • High school diploma or GED required; college degree preferred.
  • Skills & Abilities
  • Strong organizational and time management skills, with the ability to follow a checklist of daily tasks and complete them in a timely manner.
  • Ability to plan and prepare meals for multiple residents while adhering to dietary needs and budgetary constraints.
  • Physical Requirements
  • Ability to lift up to 40 lbs.
  • Ability to stand, walk, and move about for up to 75% of the shift.
  • Ability to bend, stoop, and use office and kitchen equipment.
  • Additional Requirements
  • Reliable transportation and a valid driver’s license.
  • Flexibility to work evening, weekend, and on-call shifts as needed.
  • A criminal background check is required.
